Medical Cards - Not Bracelets

Following up on last zoom support session, couldn't get photos to upload (tried all ways) but was asked to write what they say.

So I have both medical braclets and Hopkins gives their patients laminated yellow cards, the exact size of a driver's license - if you all want to try to make your own.

My front Liver and Kidney says

Please be aware that I have had Liver (Kidney) Transplant, if I am at an emergency department at a hospital other that (Hopkins) please contact (Team) via this access line (number). Than goes on to say where to contact at Hopkins if I come in through Hopkins.

Backside says

NO IV contrast without Transplant Attending or Fellow level input. Do not give fluid resuscitation liters without Transplant Attending or Fellow input. Do not give NSAIDs without Transplant Attending or Fellow input. Vascular Access concerns should be considered. PICC Lines should not be placed on transplant patient without Transplant Attending or Fellow input and on the Kidney card it says no PICC Lines at all.

This was given by teams so not something available to order. But I recently had surgery and I had to remind all the doctors involved since it wasn’t those organs related that I couldn't take ibuprofen at all as a liver patient which I don’t know why it didn't get written all over chart so that was a bit concerning.

    On the front at the end you said for Kidney not all. Where do you put not all?

    sorry - no PICC lines for kidney patients at all.
